What is USMLE® Step 3 and when should residents take it?
USMLE® Step 3 is the final exam in the United States Medical Licensing Examination series, following USMLE® Step 1 and Step 2 CK, and the last requirement for full medical licensure in the US. Unlike the earlier Steps, it is taken during residency and tests your ability to apply medical knowledge to real-world patient management.
Most residents sit Step 3 during PGY-1 or PGY-2. USMLE® Step 3 registration is managed through the FSMB and NBME. You must apply through both organizations before scheduling your exam dates via Prometric. It is mandatory for full licensure in most US states, and some programs require a passing score before a certain year of training.
USMLE® Step 3 format: MCQs and Computer-based Case Simulations (CCS)
USMLE® Step 3 is a two-day exam administered by Prometric. Day 1 covers 232 multiple choice questions. Day 2 combines 180 MCQs with 13 Computer-based Case Simulations (CCS), where you manage a patient in real time without predefined answer choices.
